Mahtomedi Youth Hockey

BEFORE YOU BEGIN:  If you have coached previously, for credentials verification purposes, you must register yourself exactly as your name appears on your CEP card. Click this link to check your CEP registration if you are unsure of the name you used when you first applied: USA HOCKEY CEP LOOK UP.  

Coaching credentials will be verified online this year, eliminating the need for cards, but names must match for data to pull correctly.   If this is your first time coaching, please register with your full name.

Here is a List of the Coaching Registration and Certification Requirements:

• Register with USA Hockey - Annually - A link to USA Hockey can be found below
• Register online with MYHA - Annually - Sign In at the bottom of this form to begin registering
• Complete the MN Hockey Background Screening - Every 2 years
• Have a current Coaching Education Program (CEP) Certification or register for and attend a course by 12/31
• Complete the Online Age Specific Module for every level that you would like to coach. 
• Complete SafeSport Training - Annually
• Complete Concussion Training - Every 2 years
• Complete and submit the Concussion Training Certification Form - Every 2 years after completing concussion training

New this year - If you are applying for a head coaching position with MYHA, you can do so online in this registration system. MYHA has eliminated the paper coaching application. 

You will also need to complete an interview with MYHA. Interviews will be held in late August. Dates and times TBD.

Registration and all paperwork are due August 31, 2023 for all travel team head coach applicants.
Mite, U6 and U8 coaches are exempt from this deadline and in person interviews.

Each Coach must register first with USA Hockey and then return to this Registration Session to sign up for the 2023-24 Season.

• Step 1:  CLICK HERE to go to USA Hockey Registration and register. Be sure to PRINT and SAVE your confirmation code(s). Remember each coach/player must have their own unique USA Hockey confirmation number. Returning coaches - Please make sure that you enter full your name EXACTLY as it is on your USA Hockey CEP Records when you register with them or you will not get credit for your previous training. 
• Step 2:  Return to this registration and proceed. Later in this registration session you will need to enter the USA Hockey Confirmation Code that you received in Step 1.
